OVERVIEW: Reporting to the South Care Supervisor, the South Case Managers provide intensive support, quality service measures, and service direction for Care Providers responsible for children and youth served by the organization. This is an opportunity to be a part of a holistic service team centered on creating and supporting positive and healthy experiences for children and families.
 
KEY RESPONSIBILITES:

  • Facilitate pre-service orientation for new Care Providers.
  • Establish a positive and supportive relationship with Care Providers, youth in the homes, and the support team through being a regular visitor to the home and maintaining ongoing communication by phone and via email.
  • Conduct intake care planning meetings with guardian agency staff and Care Providers and conduct regularly scheduled care planning update meetings. 
  • Based on your ongoing awareness of and connection to homes, complete ongoing care home documentation and assess and complete required licensing documentation:
    • Home Studies and Foster Home License Applications, as needed. 
    • Annual Reviews.
    • Addendums, as needed. 
    • Foster Care Licensing Closures, as needed.
  • Maintain an accurate understanding of the services provided in each home and effectively communicate with the Program Manager and guardian agency.
  • Collaborate with guardian/workers in delivering care plans for each child and/or family, while remaining focused on child’s wholistic needs.
  • Collect and review monthly reports and incident reports for each home, identify challenges, strategize with the home, and help care teams to implement solutions. 
  • Liaise with Care Providers to ensure information is effectively communicated. 

 QUALIFICATIONS:
 
  • Bachelor of Social Work (BSW) degree or equivalent (similar education and experience working in the field will be considered). 
  • Minimum of (3) years of work related experience. 
  • Knowledge in the areas of the impacts of colonization and intergenerational trauma.
  • Knowledge of the Child and Family Services (CFS) sector and legislation.
  • Good understanding of foster care standards and related licensing paperwork.
  • Demonstrate knowledge and/or understanding of Indigenous perspectives, First Nations culture and traditional philosophies.
  • Experience developing and supporting a strong, respectful team-based approach to service delivery.
  • Experience working with families and navigating complex situations.
  • Excellent conflict resolution skills. 
  • Excellent communication, organization, problem solving and team skills.
  • Strong proficiency in Microsoft Office, word processing (MS Word), Spreadsheets (Excel), E-mail (Outlook), and Internet, including ability to type.
  • Ability to speak Anishinaabe is an asset. 
  • Valid MB class 5 Drivers license with access to a vehicle is preferred; acceptable Driver’s Abstract required.
  • Must have a clear criminal record, child and adult abuse registry checks, and prior contact check required.
  • Ability to travel is required for this position.
