Dakota Wicozani Tipi Inc. (DWT) is seeking a compassionate, skilled, and community-oriented Community Health / Diabetes Nurse to join our Nursing Team. This multi-role position plays a key role in supporting the health and wellness of DWT community members through culturally grounded, community-based nursing services with a strong focus on diabetes prevention, management, and chronic disease care.


Position Summary

Under the administrative supervision of the Health Director and the clinical supervision of the Public Health Nurse, the Community Health / Diabetes Nurse is an integral member of the DWT Nursing Team. This role is responsible for delivering community health and diabetes nursing services, including health promotion, disease prevention, diabetes education and management, foot care, and maternal and child health services.

The Community Health / Diabetes Nurse works closely with individuals, families, and community partners to promote healthy lifestyles, support chronic disease management, and improve overall community wellness. The position emphasizes culturally safe, community-driven programming that supports holistic health across the lifespan.


Duties and Responsibilities

  • Provide community-based nursing services focused on health promotion, disease prevention, and chronic disease management
  • Deliver diabetes prevention, education, self-management support, and treatment services to individuals, families, and community groups
  • Provide foot care services and education related to diabetes and chronic disease prevention
  • Promote healthy eating, physical activity, and lifestyle changes through individual counselling and group programming
  • Provide maternal, prenatal, postnatal, infant, and early childhood health education, screening, and support
  • Develop, implement, and participate in community health promotion and prevention programs, workshops, and events
  • Conduct client assessments, case management, referrals, follow-up, and documentation
  • Collaborate with multidisciplinary health teams, community members, Elders, and external service providers to coordinate care
  • Maintain accurate and timely client records, program documentation, and data collection in accordance with DWT policies, professional standards, and funding requirements
  • Support culturally safe care by participating in community events, cultural activities, and ceremonies
  • Demonstrate flexibility and provide backup support to other nursing team members as required
  • Apply strong organizational, leadership, problem-solving, and interpersonal skills
  • Perform other related duties as assigned

Qualifications

  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) in good standing with the appropriate professional licensing body
  • Training and certification in Foot Care
  • Experience or demonstrated competency in community health nursing and diabetes care
  • Strong assessment, chronic disease management, education, and case management skills
  • Ability to plan, coordinate, and facilitate community programs and workshops
  • Strong communication and relationship-building skills
  • Knowledge of and respect for First Nation culture, traditions, and community values

Additional Requirements

  • Current First Aid and CPR certification
  • Current Criminal Record Check, Vulnerable Sector Check, and Child Abuse Registry Check
