Home and Community Care Coordinator
Permanent Position
Position Purpose: To actively work with the members of SCTC bands in the planning, development and implementation of the FNIHB Home & Community Care Program. The Tribal Home & Community Care Coordinator will provide consultation and recommendation for the establishment of processes to support the delivery of Second Level services and activities required for the operation and safe delivery of the Home and Community Care Services to First Nations.
Roles & Responsibilities:
· Assist community groups in the planning and development of their Home & Community Care proposals such as home and community care assessment, home care plans, training plans, and capital plans and as requested.
· To assist, participate and recommend the development and approval of the capital, training, planning, implementation and evaluation plans for the region.
· Responsible for the on-going monitoring and support of community care programs continuing progress through community visits, meetings, and workshops.
· Orientate First Nations to the responsibilities and requirements of the contribution agreement for the Home & Community Care Program, including goals, objectives, financial controls and reports, to assist to increase their capacity to develop and implement programs and provide information.
· Participate in all planning and implementation and regional committee meetings to report on project status and/or any special circumstances happening in the Tribal Council area.
· Collaborate and network with representatives of the various federal departments, provincial governments, professional associations and other non-government organizations for the purpose of single-window access to all continuing care services, i.e. ISC.
· Analyze and interpret regional, provincial, health, environmental, and social trends, with a view to contributing to the planning, implementation, and evaluation of Home & Community Care policies and programs for the Manitoba Region.
· Create/develop a comprehensive communication plan that promotes effective working relationships throughout the process at the regional, provincial, tribal council and community levels.
· Collaborate with key health services personnel in First Nations & Inuit Health for the purposes of program planning, monthly reporting, and evaluation of other program issues in the Home & Community Care Program.
Qualifications & Experience:
Minimum of Registered Nurse Diploma or Baccalaureate Nursing Degree. Must be registered with CRNM and in good standing. Must have demonstrated expertise in home and community care planning, programming, and community development.
· Knowledge of scope of practice issues, liability and standards of the nursing profession.
· Knowledge of other linkage with the H&CC program.
· Previous working experience with First Nations and a demonstrated understanding of the particular health & social issues.
· Should have years of experiences with the H&CC program.
· Knowledge of First Nation Culture and Language is an asset.
· Excellent interpersonal & communication skills.
· Self-directed and the ability to work independently.
· Strong organizational skills.
· Valid Class 5 Driver’s License, with availability to travel.
· Advance foot care certification, First Aid and CPR.
· Excellent computer skills, knowledge of H&CC tools.
