Reporting to the Post Majority Supervisor, the Direct Service Worker provides case management and individualized support to young adults aged 18–25 who have aged out of care. The DSW works collaboratively with participants to identify their goals and needs, supporting areas such as housing stability, education and employment planning, life skills, cultural connection, and overall wellness. Through relationship-based practice, the DSW coordinates services, advocates for resources, and helps young adults navigate systems while respecting their autonomy and voluntary participation.
 
RESPONSIBILITES: 
 
·         Reaching out to eligible former young adults who have aged out of care who require voluntary agency support.
·         Advocate for the young adult’s individual or group needs in urban or on-reserve communities.
·         Complete a comprehensive assessment of the young adult’s needs and supports.
·         Support young adults to develop, monitor, and evaluate individualized and self-identified goals and life plans with timely and ongoing wrap around supports.
·         Provide support for young adults to access financial supports, educational, and professional development opportunities; safe, stable, and comfortable housing; physical, mental, and social well-being; connection with land, culture, language, family, and community.
·         Support young adults with meaningful repatriations with their families and communities.
·         Working in and intervening with crisis situations in an appropriate and timely manner.
·         Participating in Agency events, training and working flexible hours as required.
·         Group facilitation; team building and decision-making; taking initiative; problem-solving, leading, planning, coordinating, and facilitating meetings; de-escalating conflict; and providing solutions.
·         Excellent case and file management skills; stress management; time management; and documentation detail.
·         Working in a team-based environment as well as independent responsibilities.
·         Willing and able to travel to designated First Nations communities and rural areas as required.
 
QUALIFICATIONS:
 
·         Bachelor of Social Work (an equivalent combination of education and experience may be considered).
·         A minimum of two (2) years’ experience in front-line case management with Children-in-Care and families.
·         Experience conducting investigations; gathering and analyzing information to implement appropriate action(s).
·         Demonstrated working knowledge of and familiarity with: 
o   Indigenous culture and an understanding of the rural cultural environment.
o   The Child and Family Services Act; The Adoption Act; An Act respecting First Nations, Inuit and Métis children, youth, and   families; Indigenous child welfare practices; First Nations history; and colonization.
o   Crisis intervention; child protection investigations; high-risk situations assessments.
o   Substance abuse; family violence; child abuse/neglect; grief and loss issues; and community resources.
·         Excellent verbal and written communication skills (specifically to documentation and report writing).
·         Working knowledge of Microsoft Office (i.e., Word, Excel, and Outlook), CFSIS, and IM.
·         A valid driver’s license; a reliable vehicle.
·         Satisfactory Criminal Record; Child Abuse Registry; and Prior Contact Check.
