TRIBAL DIABETES COODINATOR - DAKOTA OJIBWAY HEALTH SERVICES
Under the direction and supervision of the Dakota Ojibway Health Services (DOHS) Clinical Team Lead the Tribal Diabetes Coordinator’s (TDC) goal is to reduce type 2 diabetes among First Nation residents in DOTC communities by supporting health promotion and primary prevention activities and services delivered by community Aboriginal Diabetes Initiative (ADI) workers and health service providers.
The TDC will work in partnership with provincial and federal health service providers as well as other health promotion organizations such as the National Indigenous Diabetes Association to facilitate a range of primary prevention, screening and treatment programs. Through these activities, the TDC supports prevention, health promotion, screening and care management initiatives that are community-based and culturally appropriate.
KEY RESPONSIBILITIES:
•Creating supportive environments and increasing the practice of healthy behaviours through improved access to healthy food and promotion of healthy eating, physical activity, and healthy body weights.
•Increase awareness through workshops in community with a focus on diabetes, diabetes risk factors and complications, and approaches to prevent diabetes and associated complications among all First Nations people.
•Train community ADI paraprofessionals and assist in the delivery of workshops. Increasing community ownership of diabetes programs and capacity to prevent, delay and manage diabetes.
•Develops the public health skills and capacities of community ADI paraprofessionals and empower them to implement ADI programming at the community level.
•Participate in the early detection and screening for complications of diabetes in First Nations communities.
•Increasing knowledge and share information to inform community-led, evidence-based activities in First Nations communities.
•Develop partnerships by participating on committees, where needed, to maximize the reach and impact of primary prevention and health promotion activities.
•Participate in community Case Management model such as Circle of Care committees.
•Provides support and resources (such as healthy living initiatives, staff education, and client education guidance and materials) to local ADI workers and community health staff.
•Develop yearly workplan and budget.
•Develop and submit proposals for DOHS and communities, when necessary.
•Gather data on diabetes regional and nationally to reflect expansion of the diabetes program at the Tribal Council and Community levels.
•Participate in community health fairs.
•Assist in all other functions/duties deemed necessary by the DOHS Clinical Team Lead. The DOHS Clinical Team Lead may add duties to assist other program staff, as an incumbent duty, within reason, for administration: i.e., Director of Health, Assistant Health Director, Department Managers, etc.
QUALIFICATIONS: (including, but not limited to)
•Professional designation: nursing, dietitian or allied health professional with 2 or more years of experience in program implementation management; and recent work in chronic disease including primary prevention.
•Certified Diabetes Educator certification and knowledge of Diabetes Canada’s Clinical Practice Guidelines considered an asset.
•Knowledge, sensitivity and awareness of the history, culture and unique needs of the Indigenous population.
•Excellent written and oral communication skills,
•Experience with Power Point and presentation skills
•Proficient computer skills (Microsoft Word, Excel)
CONDITIONS OF EMPLOYMENT:
•Requires valid Class 5F Driver’s license.
•Requires access to a reliable vehicle meeting all safety requirements and must be able to travel extensively throughout region.
•Requires a Child Abuse Registry Check; Adult Abuse Registry Check; Criminal Record Check with Vulnerable Sector Screening
•Requires an active Practice Registration and must be in good standing with the appropriate nursing institution of Manitoba.
